Before you begin

Describe what happens in daily life

Answer based on safe, reliable, routine function—not what might be possible once, on a best day, or only with unusual effort. The goal is completeness, not a higher score.

Identify hands-on assistance, cueing, supervision, and monitoring.
Describe frequency and what occurs without caregiver involvement.
Include safety risks, equipment use, and medical complexity.
Print the final summary for preparation or discussion.

Physical Function

Record the assistance required with personal care, mobility, transfers, and continence.

Domains 9–16

Cognitive, Behavioral, and Medical Complexity

Record supervision, communication, decision-making, memory, treatments, monitoring, and diagnoses.

Daily care context

Caregiver Involvement and Safety

Capture care that may not be visible in medical records or a single clinical encounter.

Can the Veteran safely remain alone? How often does another person intervene?

Common caregiver interventions

Reminders, cueing, or redirection Supervision or standby assistance Hands-on physical assistance Medication setup or oversight Medical monitoring or device oversight Behavioral or emotional de-escalation Appointments, scheduling, or daily structure Fall prevention or other safety intervention
